3 Simple Steps
Step 1:
If you have been in a motor vehicle accident, you are entitled to up to 21 treatments through the Insurance Act of Alberta. Call your auto insurance company and register your claim, whether you are at fault or not.
- You do not need to see a medical doctor for a referral for treatment.
- Another car does not have to be involved.
- Be aware that many insurance companies have two claim adjusters, one for your vehicle and one for bodily injuries.
- Your rates will not increase with a bodily injury claim.
- It is not uncommon for symptoms to appear several days after the accident. If you initially informed your adjuster you had no symptoms, contact them immediately and tell them you are now seeking treatment.
Step 2
Contact our office immediately or book online HERE. You can begin treatment immediately even if you are still waiting to hear back from your insurance company.
- Your benefits will depend on the type of injury you have.
- We will be able to direct bill the insurance company for all chiropractic services.
Step 3
Download, print and complete these 4 forms (10-15 minutes):
- Patient Insurance Information
- AB‐1 Form *Sections 1, 3, and 6 only
- AB-2 Form *Sections 1 and 7 only
- Patient Symptoms Checklist
- Fill out all 4 forms, have them completed before you arrive. Be aware of which sections are to be filled out.
- Dr. Jodi will forward them to your insurance company.
- Timely and proper care will result in faster recovery and help prevent more serious long-term injuries.
Whiplash Associated Disorder
The most common injury resulting from an automobile accident is whiplash, occurring from the sudden acceleration (or deceleration) of the head and neck during collisions.
Whiplash can be so severe that the most menial daily tasks can't be carried out or it can be a mild annoyance that prevents you from turning your head to the side. Regardless of the intensity of the injury, it isn't something that should be left untreated. Some cases (even the mildest) can create muscle stiffness that causes long-term chronic pain.
